OUR HIPPA POLICY

HIPPA stands for the Health Insurance Portability and Accountability Act. This Act was introduced to ensure continuity of health coverage in the event of changes in insurance carriers so that coverage would not be denied because of pre-existing medical conditions.
Since its introduction the scope of the Act has been enlarged to address the privacy and the security of medical information.

PRIVACY : The privacy regulation protects and individual's right to control access and disclosure of his protected, individually identifiable health information.

SECURITY : The security regulations require that organizations control the means by which health information remains confidential.


 

How we collect information on your child :

We obtain your personal demographic information and that of your child: such as, your birth date, social security number, and medical insurance information. We will require you to fill out a form and also obtain copies of your driver's license and insurance card.


Why we collect this information :

We collect this information so that we can treat your child's medical condition and obtain payment from you or your insurance


Who has access to this information :

Any person that you designate in writing, anyone directly involved in your child's medical care directly (nursing, physicians, other medical offices etc) or indirectly, through creating and maintaining your child's medical record, and those entities that need your child's information to process and submit health care claims in order to obtain payment for services. Government Oversight agencies, Judicial and Administrative Proceedings, Law enforcement Agencies, Coroners and Medical Examiners, and Organ Procurement Organizations may obtain copies of your child's PHI. We have no jurisdiction over such entities.

How we protect this information :

Desert Ridge Children's Center will release your child's information only to those people who you authorize or who need the information in order to provide for the medical care and needs of your child. We maintain physical, electronic, and procedural safeguards to ensure that only those involved in your child's care have access to that information.

Your Rights :

You have the right to inspect your protected healthcare information. You have the right to request and amendment to any errors you may find in your child's record. If you leave this practice, your child's medical history will continue to receive the same protection.

Comments/ Complaints :

If you have any questions or concerns regarding our privacy practice, you may call the privacy officer at this practice at 480-585-5200.

We take all your complaints extremely seriously. Necessary investigation will be performed and action taken, depending on the nature of the complaint.

Desert Ridge Children's Center reserves the right to amend our privacy policy as dictated by law, without sending you a copy of the amendment. Any changes to this policy will be posted in our office. A more comprehensive policy is available for review at our office.
